Paperwork done before they land

Send each guest a link and they fill in their details, the diver medical questionnaire and the liability release on their own phone. DiveOS uses the 2020 Diver Medical Participant Questionnaire, so a yes only sends someone to a doctor when the follow-up questions say it should.

What it does

  • The current RSTC/UHMS questionnaire, with the follow-up boxes. A diver who had grommets as a child answers the four ear questions and dives — the old single-page form sent them to a physician.
  • A signature is the typed name plus the time, the IP address and a SHA-256 hash of the exact text that was signed.
  • Staff can fill any form on a counter tablet for the guest who did not, and it is recorded as having been done at the counter.
  • Reminders go out seven days before arrival if forms are outstanding, and the day before with what to bring.
  • Form status per guest — not sent, sent, partly done, complete, needs clearance, cleared — on the booking and on today's arrivals.
  • Decide per centre whether a missing release blocks check-in or only warns.
  • A walk-in becomes a guest, a booking and a check-in on one screen.
